  • List all members in your household even if they are not Catholic.
  • Do not add multiple members under one section; list each member's information separately.
  • Adult children (25+) should register separately (unless they are students or have special needs).
  • If there are multiple families in your household, register each family on a separate form.
  • If you are currently registered at another parish, please contact them to un-register.
